Each entity provides home health care services, which may include both skilled medical services (such as nursing, therapy, and home infusion therapy) and non-medical personal care services (such as assistance with daily living, companionship, and homemaker support), delivered by appropriately licensed and qualified professionals. For purposes of this Privacy Policy, “Gracefield Home Care,” “we,” “our,” or “us” refers collectively to all entities operating under the Gracefield Home Care brand, and where context requires, to the specific state-licensed entity with which you engage. To the extent a provision must be interpreted under a particular state’s law, it shall apply only to the entity operating within that state.
Scope
This Policy applies to information collected through the Website, through state-specific online forms that route directly to the corresponding entity, by phone, email, or other electronic communications arising from Website submissions. This Policy complements—but does not replace—any HIPAA Notice of Privacy Practices (“NPP”) that we provide at intake or upon request. Where this Policy conflicts with an NPP as to Protected Health Information (“PHI”), the NPP controls.
Information We Collect
Identifiers and Contact Information: name, address, email, phone, and similar details.
Health and Medical Information (PHI): information you or your authorized representative submit to request or coordinate services, such as conditions, medications, functional status, physician information, insurance details, schedules, and preferences.
Professional/Employment Information: if you apply for employment, licensing, credentialing, background, and eligibility details as allowed by law.
Technical Information: IP address, device type, and basic telemetry necessary to operate the Website. We do not use non-essential cookies or third-party analytics on this Website at this time. Our hosting provider (including Squarespace) may set strictly necessary cookies to deliver core site functionality.
How We Use Information
To respond to inquiries, schedule assessments, and provide, coordinate, and manage care.
To verify eligibility, arrange staffing, billing, and payment.
For quality assurance, safety, compliance, and training.
To communicate with you about services, updates, and required notices.
To comply with law, regulation, court orders, and government requests.
HIPAA and PHI
When we create, receive, maintain, or transmit PHI as defined by HIPAA, we do so in compliance with applicable federal and state laws. We may use and disclose PHI for treatment, payment, and health care operations without your authorization, and for other purposes permitted or required by law. Uses or disclosures not described in our NPP will be made only with your written authorization. You have rights regarding your PHI, including to request access, amendment, and an accounting of disclosures, and to request restrictions and confidential communications, as described in our NPP.

To our workforce members and service providers who need the information to perform services and who are bound by confidentiality obligations.
To health care providers, payors, and other third parties as necessary for treatment, payment, and health care operations.
To comply with law, including public health activities; to avert serious threats to health or safety; for oversight; and as otherwise permitted by HIPAA and state law.
In connection with corporate transactions limited to the services in a given state, subject to applicable law and any required notices.
Children’s Privacy
The Website is not directed to children under 13. We do not knowingly collect personal information online from children under 13 without appropriate authorization.

Changes to This Privacy Policy
We may update this Privacy Policy from time to time. When we do, we will revise the “Effective Date” above. Material changes will apply prospectively. Your continued use of the Website after an update constitutes acceptance of the revised Policy.
